Alcohol consumption, hepatic iron load and the risk of amoebic liver abscess: a case-control study.

چکیده
OBJECTIVE It is unknown why most amoebic liver abscess (ALA) cases occur in alcohol drinkers. In experimental studies, the presence of 'iron' potentiates the in-vitro growth of Entamoeba histolytica (E. histolytica), and is also known to increase its in-vivo invasiveness in animal infections. Chronic alcoholism increases the hepatic iron deposition. We hypothesized that ALA occurs more commonly in livers with a high iron load as in alcoholics. To test this hypothesis we compared the levels of iron between ALA and non-ALA cases belonging to alcoholic and non-alcoholic groups. METHODS Out of a total of 48 ALA cases, 34 (70%) were alcoholics and 14 (30%) were non-alcoholics. After applying exclusion criteria, serum iron and liver iron stores were quantified in 20 ALA cases (10 alcoholic and 10 non-alcoholics) and compared with 20 non-ALA cases (10 alcoholics and 10 non-alcoholics). RESULTS All patients of ALA had serum iron values within the normal range but higher than non-ALA cases. In the liver tissue, most patients with ALA had higher (grade II or III) iron deposition, than non-ALA cases (mostly grade I). Thus, patients with ALA, with or without alcohol indulgence, had higher iron levels when compared to the non-ALA cases. CONCLUSION It appears that the higher incidence of ALA in alcoholic livers is possibly due to their higher iron content.

An unusual case of amoebic liver abscess presenting with hepatic encephalopathy: a case report.
Amoebic liver abscess (ALA) with jaundice and encephalopathy is a rare occurrence and has been recognised and studied more frequently in recent years. We present a case of massive ALA presenting with jaundice, hepatic encephalopathy, and septicaemia that was treated successfully with percutaneous drainage of the abscess, right-sided chest tube insertion, and anti-amoebic therapy.
متن کاملEpidemiology and factors associated with amoebic liver abscess in northern Sri Lanka
BACKGROUND Clinically diagnosed amoebic liver abscess (ALA) caused by Entamoeba histolytica has been an important public health problem in Jaffna district, northern Sri Lanka for last three decades. In order to draw up a control strategy for elimination of this condition, knowledge of its epidemiology and factors associated with this condition in the local context is vital. METHODS All clinic...
متن کاملPathophysiology of jaundice in amoebic liver abscess.
Jaundice in patients with amoebic liver abscess is a frequent occurrence. However, the pathophysiology of jaundice in these patients is not fully understood. Hepatic necrosis leads to damage to bile ducts as well as various vascular structures, which in turn leads to biliovascular fistula and jaundice. We studied the mechanism of jaundice in patients with amoebic liver abscess. We prospectively...
متن کاملAmoebic hepatic abscess--potential causes of delay in diagnosis.
Delay in diagnosis can be serious in amoebic hepatic abscess, which is readily treatable. Two cases are presented to illustrate potential causes of delay. The first case had never been to an endemic area, apparently contracting the infection from his wife. The second case had negative serological tests.
متن کاملSpecific circulating immune complexes in amoebic liver abscess.
A micro enzyme-linked immunosorbent assay was used to detect the presence of amoebic antigen in polyethylene-glycol-precipitated circulating immune complexes (CIC). A cutoff dilution of greater than 1:160 for the precipitates was taken to be of clinical significance. Among the patients with amoebic liver abscess, 93% (14 of 15 confirmed cases) had indications of amoebic antigen in CIC. In 57% o...
